THE MOST IMPORTANT STANDARDIZED FORM A VETERAN WILL EVER HAVE
Your DD
form 214 is your lifeblood to receive all the benefits that you may be
entitled to as a veteran. It is the starting
point for your education, employment, healthcare, disability compensation and
other Federal and state veterans programs.
It is important to have your copy… It is important that your copy accurately reflects your actual service including deployment, training, enlistment dates, and awards/medals.
Campaign awards and medals are often approved after you’ve fulfilled your military commitments and received your DD form 214. Many of your initial and additional benefits are dependent upon an accurate DD form 214.
For
example: If your DD form 214
is inaccurate, you may not be considered eligible for the PA Bonus for
Veterans of the Persian Gulf Conflict (August 1990 to August 1991) and
possibly receive compensation up to $525. If you were declared a prisoner of
war you may be entitled to a one-time benefit of $5000. (For applications for this benefit please

DD form 149 Application for Correction of Military Record
Please submit all supporting paperwork (copies of orders, award letters, ect.). This evidence will be reviewed to determine if your DD-214 must be updated.
DD form 293 Application for Review of Discharge

After you have completed and submitted the DD-149 or DD-293,
your request will be approved or denied.
Upon approval, a DD Form 215 is used to correct errors or additions to a
DD Form 214 that were discovered after the original had been delivered and/or
distribution had been made. You will
receive a copy of the updated DD form 214/215.
Copies will also be distributed to the appropriate Federal and State
agencies.
